In 1970, in Santa Monica, John Form, a doctor, gave his pregnant wife, Mia, a rare vintage porcelain doll as a gift for their first child. Mia places it in her collection of dolls in the room of their future child.
That night, Mia is awakened by a scream coming from the neighbors, the Higgins. Mia and John run to the Higgins' home. As John comes out of the neighbors' house, with blood on his clothes, he asks his wife to call an ambulance. While Mia calls the police, she is attacked by the Higgins assassins. John arrives just after his wife has been stabbed in the stomach and tries to neutralize the attackers. Police arrive and kill the man as the woman commits suicide by slitting her throat inside the future baby's room, holding the porcelain doll. Police identify attackers as the Higgins couple's daughter Annabelle and her unidentified boyfriend, both members of a satanic cult
In the days following the attack, a series of paranormal phenomena occurred around the Form residence. John decides to throw the doll away because it brings back bad memories to Mia. While John is away, a strange fire breaks out in the house. Following the incident, Mia gives birth to a healthy baby girl. She and John name their child Leah. Mia no longer wishing to return to their house, the family rents an apartment in Pasadena and Mia finds Annabelle in one of their boxes, which she chooses to keep after consideration. Another series of paranormal events plague the lives of Mia and her daughter. During a storm, as Mia goes down to the basement of the building, she is alerted by crying babies. That's when she comes face to face with an enigmatic figure with black skin and yellow eyes who begins to torment her and goes as far as chasing her down the stairs, but Mia manages to escape him.
Mia calls Detective Clarkin back to gather information on Annabelle and the sect that the Higgins assassins were a part of. She learns that the assassins practiced Satanism and rituals in order to summon demons or the devil. With the help of a bookseller, Evelyn, living in the same building, Mia learns more about satanic rites. In particular, she discovers that the summoned demon wishes to take a soul. Back home, Mia and Leah are attacked by the demon who reveals himself while manipulating the doll. Mia and John contact their parish priest, Father Perez, who informs them that demons sometimes attach themselves to inanimate objects to achieve their goals and that a human soul must be offered for a specific purpose. Without hope of exorcising the demon from the doll, Father Perez decides to kidnap it to ask the Warrens for help with an investigation. But before he can enter the church, the demon appears as the Higgins' daughter and violently attacks her. He takes the doll with him, leaving the unconscious priest at the doors of the church.The priest is hospitalized and, when John is at his bedside, Father Perez warns him that the true intention of the demon is to claim Mia's soul. Meanwhile, as Evelyn visits Mia, the demon materializes as Father Perez and shows up at the apartment door and begins to scream. Afterward, Evelyn tries to help Mia evacuate the apartment with Leah, but the demon suddenly appears in her true form and brutally throws herself at Evelyn to get her out of the apartment before kidnapping Leah. While Mia, terrified, begins to search for her child, the demon makes Mia understand that he will return Leah to her in exchange for her soul. To spare her daughter, Mia tries to jump out the window with the doll, but John arrives with Evelyn in time to stop her. With the demon still clamoring for a soul, Evelyn decides to end her own life in Mia's place as atonement for causing a car accident that resulted in the death of her daughter Ruby several years ago. The demon disappears with the doll, as Leah reappears in her crib. Thus, the whole family is reunited to continue their life.
Six months later, the doll is purchased from an antique store by a mother as a gift to her daughter Debbie, one of the nursing students featured in the film's prelude as well as that of Conjuring: The Warren Files.
